Not necessarily. But if it's from the AC, it likely is.
Flaxen, like all our gene colors, has an array of base colors the game can give it, all, of which, are also genetically inheritable to offspring. Flaxen can appear as a pseudo-flaxen, a foal that doesn't appear to have any flaxen, from two flaxen parents, all the way to a white-flaxen, a flaxen that appears more like the silver gene, rather than the regular, washed-out, reddish-tinge that they generally appear to be.

You will only, usually see certain color variations from the AC, so if something appears really unusual from the AC, it's highly suspicious of a glitch.

Not necessarily - flaxen can appear very white. If the tail is also white, it might not be a glitch.

There is one example of almost white mane and tail as a youngster (now sooty has somewhat spoiled the effect) :
